Registration Kits
Ordering Your Registration Kits
It's easy! Order your Registration Kits with pre-printed forms EARLY. We need a minimum of six weeks prior to your first registration event to print your order. Remember to ask for your pre-printed forms. They are efficient and save precious time. These forms have historical player and volunteer data already filled in and just need a signature to confirm. As of August 1, 2012, the kits are provided as stated below.
Registration Kit includes the following:
- Pre-Printed Forms (Subsidized)
- Player ($0.22/form)
- Volunteer ($0.14/form)
- Youth Volunteer ($0.14/form)
- Blank Forms (Subsidized)
- Player ($2.50 for pack of 50)
- 18-yr old Player ($1.25 for pack of 25)
- Volunteer ($2.50 for pack of 50)
- Youth Volunteer ($1.25 for pack of 25)
